• Please print out the following forms, complete them prior to your appointment, and bring them with you to your first visit. All Medicare patients must complete the private contract. If you do not have a printer or are unable to complete them prior to your visit, you will be asked to complete the packet at the time of your first visit:
  • Please arrive on time for your scheduled visit. 
  • If you arrive more than 10 minutes late to your scheduled appointment, you may be asked to reschedule your visit if Dr. Stone decides that your needs cannot be adequately addressed in the time remaining. Additionally, a late-cancel fee of $50 may be assessed.
  • Please park in The Therapy Center parking lot accessed from Sunset St.
  • Office visit charges are reasonable and very competitive! Therefore, no form of public or private health insurance is accepted.
  • No membership fee is required to establish care with Dr. Stone.
  • HSA and FSA cards accepted.
  • Payment is expected at time of service.
  • Discounted fees, payment plans, and/or bartering for goods or services available in cases of financial hardship or other special circumstances. Please discuss this option with the office if it interests you. 
  • If you need lab or imaging ordered, the cost associated with these tests will be billed to your health insurance directly by the lab or imaging center. Dr. Stone is eligible to order tests and generate referrals for patients on traditional Medicare plans (NOT Medicare Advantage). If you do not have health insurance or if you have a high-deductible insurance policy, Dr. Stone will work with you to locate available self-pay discount pricing for these items. 
  • COVID-19 triage: If you have respiratory symptoms with fever, sore throat, cough, runny nose, shortness of breath, or flu-like illness, please phone office directly for further instructions before scheduling an appointment. You may need to have lab or imaging performed at outside facilities. At this time, Remedy Clinic PC is not performing in-office testing for SARS-CoV-2 infection.
  • Please bring your own mask if you are coming into the office for a visit. You may need to wear a mask during your office visit in compliance with state and local guidelines for healthcare settings. At the present time, spring 2023, the office is mask-optional unless you have an active cough or viral symptoms, in which case mask wearing is expected.
